This addendum has been issued by the Town of Penetanguishene Public Works Department to revise and clarify selected sections within the original Tender document.  The section of the RFP that have been altered are identified with red colored font.

The Respondent, having carefully examined the RFP, and all documents relating thereto, does hereby offer in accordance therewith to supply the services, and all components thereof, as required by the Town of Penetanguishene and as outlined in the RFP.

 

THE RESPONDENT HEREBY AGREES:

1.         That this submission is subject to the acceptance of the Corporation of the Town of Penetanguishene;

2.         That, among other matters, the Highest Scoring or any submission will not necessarily be

accepted by the Town; and

3.         To undertake to provide any goods and services as required by the Town in accordance with the

RFP.

 

THE RESPONDENT HEREBY DECLARES:

1.         The Respondent acknowledges that it has reviewed the Standard Terms and Conditions and all other terms and conditions of the RFP in their entirety and acknowledges that subject to such changes as are agreed upon by the Respondent and the Town, such terms and conditions shall form the basis of the contract between the parties;

2.         By signing this submission, I confirm I have read and understood the content and requirements of

this Submission document;

3.         The Respondent has fully reviewed the information and the Specifications and/or Scope of Work

and submits this information in anticipation of the fulfillment of the Specifications and/or Scope of

Work at the price quoted;

4.         No person, firm or Town, other than the Respondent, has any other interest in the submission or inthe award for which the Submission is made;

5.         This submission is made without any connection, comparison of figures, or arrangements with, or knowledge of any other Town, firm or person submitting for the same work and is in all respect fair and without conflict of interest, collusion or fraud;

6.         This submission is made in accordance with the Municipal Conflict of Interest Act, Ontario;

7.         The submitted price, and all other terms are valid for a period of sixty (60) days from the Request for Proposal (RFP) closing date or until a Contract is signed with the Successful Respondent(s), whichever comes first;

8.         This submission has been executed by a duly authorized signing officer and the corporate seal has been affixed, where applicable.

9.         Accessibility training as required by AODA of all employees, agents, volunteers and subcontractors hired by us in completion of our work has been completed.
